Thank you for your interest in registering! We will be notified as soon as you have registered via the website. We will then register you within a few days or you will be put on the waiting list. When the registration has been processed, you will receive a confirmation email (also keep an eye on your spam box!). From that moment on we are your doctor. Then do not forget to unsubscribe from your old GP so that we can receive your medical file.

  1. Complete the registration form on the website.
  2. You will receive an email when we have registered you (from that moment on we are your GP).
  3. You request your old GP to send your medical file to us.

Huisartsen Sloterdijk is open. Anyone living in postal code 1043 or 1014 is invited to select us as your general practioner. Living means , you are registered at the GBA (Gemeente Basis administratie) of the city of Amsterdam.

Please read the message below carefully and complete the registration form.

  • You will a receive confirmation of your registration by email as soon as we have checked your details. Please contact us in 2 weeks in case you haven’t received a message.
  • You need a DUTCH phone number starting with +31, otherwise we cannot call you, and we cannot give you the portal code with 2 FA.
  • If you are registering several family members at the same time, please fill in a form for each of them separately.
  • Children need to have a UNIQUE EMAIL ADDRESS to create their own portal. This may be an alias of another mail address. However, the phone number may be shared with several family members. Anyone aged 16 or over must give their own consent to be registered.
  • we work together with the Sloterdijk pharmacy, it’s a depandance from the Ringvaart pharmacy, please register here as well https://www.netwerkapotheken.nl/apotheek-de-ringvaart  . As the pharmacy is still under construction, they will deliver the medication in your mail box on mon-wed-fri. Soon a pharma safe will come, and then you can get medication out of this machine.
  • You can give permission for your data to be shared with other healthcare providers via the website Volg je zorg. This benefits the quality of care.

Practical Information

For patients without a Dutch health care insurance, we use the NZA (Dutch Healthcare Authority) tariffs:

  • Consultation at the practice by phone or e-mail : €35,15
  • Consultation at the practice by phone or e-mail over 20 minutes: €67,01
  • Home visit: €51,08
  • Home visit over 20 minutes: €82,94

Are you unable to keep an appointment?
Please let us know at least 24 hours in advance and we will schedule a new appointment. If you do not let us know or if you cancel too late, we will charge you a late fee of €25. Please note that you pay this bill yourself and will not be reimbursed by your health insurance company.

For urgent matters outside opening hours – contact the General Practice Center at 088 – 00 30 600.
